Transportation

We provide transportation services for the Hualapai Community Members who are in need. ​Transportation is provided to individuals who have medical appointments, locally or out of the local area for shopping at a fee of $10 per 100 miles. 

We provide for:

•Discharge from Facility
•Dialysis Patients
•Dental Appointments
•Physical Therapy
•Non-medical Appointments
•Shopping
•Special Events
•Ambulatory
•Wheelchair Transportation

MEDICAL APPOINTMENTS TAKE PRIORITY over all other transportation requests.

 

To request transportation please call the Transportation Program at 928.769.4188. Please have your appointment information available OR you may fill out the Transportation Request Form and return it to the HEW at your convenience. Note: Last minute notifications do not guarantee transportation accomodations. To avoid frustration please request transportation as soon as possible.
